Introduction
Please read through this checklist carefully, as it details the types of documents you will be required to provide in support of your Other Family Migration (Aged Dependent Relatives, Remaining Relatives or Carers) application. It is important that you are able to complete as many of these requirements as possible prior to lodging your application, as this will ensure your application is processed without unnecessary delay. Please note that this checklist refers to a number of documents that are specific to China, as it assumes that the applicant is a Chinese citizen.
When organising required documents please have regard to the following:
• We only accept notarised / certified copies of official documents. In Australia, copies must be certified by a Justice of the Peace or Registered Migration Agent or a person before whom a Statutory Declaration can be made under the Statutory Declarations Act 1959 as a true copy of the original. In China, copies must be notarised by a Public Notary Office.
• Documents in languages other than English must be accompanied by an English translation. In Australia, translations must be performed by a NAATI (National Accreditation Authority of Translators and Interpreters) accredited translator. In China, translations must be performed and notarised by a Public Notary Office.

Application Forms and Fee
• Completed and signed Form 47OF Application For Migration To Australia By Other Family Members;
• Completed and signed Form 40 Sponsorship For Migration To Australia;

Visa Application Charges
• Form 47A Details Of Child Or Other Dependent Family Member Aged 18 Years Or Over (if you have any children over 18, including those from a previous marriage, or dependent family members – whether migrating with you or not). Please also provide evidence of your child's dependency on you if he/she is included in your application;
Health and Character Documents
• All applicants aged 11 years old and over are required to undergo a medical and chest x-ray examination. Applicants aged less than 11 years old are required to undergo a medical examination only. You should make your own arrangements to attend an approved immigration panel hospital to undergo these examinations only after our Office has contacted you and requested you to do so. Please refer to the "Other Family Migration - After You Apply" page for further details.
• Form 80 Personal Particulars For Character Assessment (for visa applicants aged 16 years or over);
• National Police Check Application Form (for visa applicants who have been to Australia for twelve months or more during the past 10 years). This form contains all the relevant instructions on how to obtain an Australian Federal Police (AFP) certificate. This applies only to individuals over 17 years of age;
• A notarial Certificate of No Criminal Conviction for each applicant aged 16 years or over (available from the relevant Chinese authorities). In addition, if you or any migrating dependents have been to any country other than Australia before and stayed for more than 12 months within the last 10 years, please provide a Penal Certificate from this/these country/countries. This applies only to individuals over 16 years of age. Please refer to www.immi.gov.au/allforms/character-requirements/index.htm for information on character and penal clearance requirements.
General Documentation Requirements
• Evidence of the sponsor’s Australian Citizenship or Permanent Residence, such as a certified copy of their passport or ID page or Australian Birth Certificate;
• If your sponsor is a New Zealand citizen, apart from the copy of the passport or ID page, a Form 80 Personal Particulars For Character Assessment completed by your sponsor is required;
• A notarial Certificate of Birth for each applicant;
• Notarised or certified household registration booklet (full-paged and with an English translation) for each applicant;
• Notarial relationship certificate(s) between you and your sponsor (if the sponsor is your in-law, evidence showing the relationship is needed);
• Evidence of your marital or single status. This may include a notarial marriage certificate, a single statement, all pages of divorce documents that show details of any child custody arrangements or a death certificate for the previous spouse;
• Two (2) recent passport size photographs of you and any dependent child whether migrating or not, and two (2) passport size photographs of your sponsor. Please write the name and date of birth on the back of each photo;
• A copy of your passport(s) or national identification card;
• 6 labels with your postal address written on them (if address is in China please write in Chinese script)
Specific Documentation Requirements
• If you are going to apply as an Aged Dependent Relative, you also need to provide evidence of financial support from your sponsor or your sponsor’s spouse;
• If you are going to apply as a Remaining Relative, your notarial relationship certificate must state that, “These are all the children of your parents” or “Your parents do not have any other children”;
• If you are going to apply as a Carer, you also need to provide evidence that your relative or the member of their family who needs the care has completed a Medibank Health Solutions (formerly known as Health Services Australia or HSA) assessment. This can be a certificate from Medibank Health Solutions or a letter from Medibank Health Solutions evidencing that the relevant medical assessment has been sought;
• If there is a secondary applicant (such as a dependent) included in your application, please provide the following documents:
1. Notarial relationship certificate(s) between you and the secondary applicant(s);
2. If the secondary applicant is under 18 years old, a consent letter from his/her guardian regarding his/her migration to Australia. This letter should be accompanied by a copy of the guardian’s Chinese ID Card and contact details. A notarial certificate of such a letter is also acceptable;
3. If the secondary applicant is over 18 years old, please fill in Form 47A and provide evidence showing the secondary applicant is dependent on you.
